ADMISSION
PROCEDURE
1. APPLICATION/TESTING FEE AND APPLICATION FOR ADMISSION FORM
APPLICATION/TESTING
FEE: Please pay the P200 Application and Testing Fee at the
High School Cashier's Office. Present the official receipt to
the Counseling and Testing Center where you will be issued the
Application for Admission Forms.
APPLICATION FOR ADMISSION FORM: Fill this form carefully and
PRINT or TYPE all information requested. Only forms completely
and neatly filled out will be accepted.
INCOMPLETE FORMS WILL NOT BE PROCESSED.
2.
RECOMMENDATION FORM AND ACADEMIC REPORT CARD
RECOMMENDATION
FORM: Please write your full legal name in the space provided
for at the top of the Recommendation Form and ask your Principal,
Guidance Counselor or Class Adviser to complete the Recommendation.
Provide him/her with an envelope with your name written on the
upper left hand corner. This should be submitted with your accomplished
application Form.
ENVELOPES WHICH ARE UNSEALED AND UNSIGNED WILL NOT BE ACCEPTED.
ACADEMIC REPORT CARD: Secure a photocopy of your Report Card
(Form 138B) for incoming Freshmen, For Second and Third Year
transferees, provide a Reference Copy of the Transcript of Records
for evaluation purposes and have it authenticated by your School
Registrar or Principal. Your authenticated Report Card/Transcript
should reflect your grades up to the Second or Third Grading
Period. Submit this with your accomplished Application Form.
3.
ENTRANCE EXAM
Upon
submission of your completed Application Form and other admission
requirements, you shall be issued an examination permit. Take
the entrance exam or Ateneo Qualifying Test (AQT) at Ateneo
de Davao High School on the date and time specified in your
exam permit.
4.
APPLICATION FOR FINANCIAL AID
Those
applying for financial aid must ask for a separate Scholarship/Grant-in-Aid
Application Form and fill it out correctly and completely. Requirements
for grant-in-aid application must be submitted together with
the application form for admission on the date specified by
the Admission Officer. APPLICATION FOR FINANCIAL AID DOES
NOT, IN ANY WAY, INFLUENCE ACCEPTANCE OR NON-ACCEPTANCE INTO
THE HIGH SCHOOL.
5.
TEST RESULTS
The
results of the Ateneo Qualifying Test will be posted on the
bulletin board at the main lobby of the High School Administration
Building. Results must be claimed from the Office of the Student Development Center (SDC).
(Note: Those who find it difficult to enroll on the days assigned to a year level
may still enroll up the end of May, 2009)
